For Home Health services:
Home Health is available only with Original Medicare.
You can submit a request through our form, and within a few hours we will let you know if you qualify.

For Hospice services:
Hospice services are available with both Medicare and Medi-Cal.
After you submit a request, one of our specialists will contact you to clarify details, as Hospice care is provided only for patients with specific qualifying diagnoses.
What is the difference between Hospice and Home Health?

Home Health provides medical care for patients who are recovering from an illness, injury, or surgery and are expected to improve. The goal is treatment, recovery, and helping the patient regain independence.

Hospice focuses on comfort and quality of life for patients with a serious or life-limiting illness when curative treatment is no longer the goal. Care is centered on pain management, emotional support, and dignity.
What is the difference between Medicare HMO and Medicare Original(Straight)?

With an original(Straight) plan, you can choose any doctor and use external services while still receiving all our services and cards. With an HMO plan, you can receive the provided services and cards but cannot visit outside doctors or use external services except ours
